accident/incident reporting procedure

Contact Locher Immediately. All accidents require reporting and some accidents require immediate reporting by law.

In order to do this, Locher must be notified of any accident immediately, so that we may fulfil this obligation.  All Locher Temps who are involved in an incident at work are required to complete an Investigation Report Form.

Click to download  the Investigation Report Form.

Reporting Procedure

If you have an accident, are injured or have a near miss then you MUST report it immediately to Locher and your supervisor at the client site.  This is essential in allowing Locher to direct you to appropriate medical treatment and/or to implement a safe and effective return to work program.

Temps who are injured and wish to lodge a claim MUST contact Locher immediately after the accident in injury occurs.

A claim will be investigated by a formal investigator as part of normal internal procedures.  This will involve the Temp, the client/supervisor and any other relevant parties, being interviewed by the investigator.

IT IS AN OFFENCE TO PROVIDE ANY FALSE OR MISLEADING INFORMATION ABOUT A CLAIM.

A Workers Compensation Medical Certificate must be provided to Locher to cover all time lost from work due to the workplace related injury.

Rehabilitation

An injured Temporary employee with a Workers Compensation claim must attend all medical appointments reasonably made for them. Temps are entitled to also see their own doctor for medical treatment.  Please contact Locher after any doctor's appointment to advise of your condition.

An injured Temporary employee will be required to actively participate in a rehabilitation program to return to work.  The Workers Compensation and Rehabilitation Act 1986, requires any person on a rehabilitation program/plan to give their full co-operation.

Locher will endeavour to make suitable duties available to an injured employee. If suitable duties are made available then the Temporary employee has an obligation to undertake these duties. Workers Compensation payments may be reduced or stopped if the injured person fails to undertake suitable duties when offered.
